We are partnering with Monroe County Clerk Cheryl Dinolfo to provide passport services here at First Bible on Tuesday, April 29, from 5 P.M. to 8:00 P.M. Those interested should call 392-0777 ext. 205 to schedule an appointment. Applicants applying for a passport need to present:
1) Proof of citizenship; ie: a birth certifi cate w/ raised seal or old passport
2) Proof of identity, ie: drivers licence
3) Two identical 2” x 2” photos with white back ground
4) A completed passport application form (DS-11).
The application can be obtained by going to the US Department of State’s website at
http://travel.state.gov/passport/passport_1738.html
The combined passport application and processing fee for adults, 16 years and older is $100, and for children 15 and under is $85. Photos are available on-site for an additional cost of $7 per set. We accept cash, check, visa or mastercard. For complete application requirements go to www.monroecounty.gov or call the Monroe County Clerk’s Office at 753-1600. |
